We do contact the insurance carrier to verify your coverage however, there is no guarantee of the coverage or payment until the claim has been processed. Insurance is a contract between you and your insurance company. We are not a party to that contract. This contract is between you and your insurance carrier. We will file your claim but we cannot be involved in disputes relating to your payments of benefits. You are responsible for payment in full regardless of any insurance companys arbitrary determination of usual and customary rates or failure to pay for any reason. We appreciate your understanding in this matter.

Our practice is committed to providing outstanding treatment and care therefore our fees our based on commensurate rates for this service. Payment is due at the time services are rendered. We accept Visa/Mastercard, Discover, American Express, Check or Cash. In addition, we also offer pre-approved extended billing through Care Credit. You can go to their website at (www.carecredit.com) to be pre-approved. Our fee schedule varies from tooth to tooth and also from the complexity of the treatment.
